Everyone diagnosed with dementia living in Cornwall has access to a Dementia Specialist Nurse Practitioner, whose job is to support people with dementia in the local community at a time of need, or with regular monitoring. They can visit people at home. If you have not been contacted by the Dementia Specialist nurse then speak to your GP or Social Prescriber who can contact them on your behalf.

Links to local memory cafes and local groups, support and advice, plus local connections. If you are newly diagnosed with dementia, you should have a named support worker and regular review, according to need. Please speak to one of our Social prescribers if this hasn't yet happened, or if you need additional help for yourself or for someone you are caring for.
