The role of Clinical Pharmacy within primary care is to pro-actively help patients stay safe and well, and out of hospital as well as helping to reduce the demands on general practice.

Clinical Pharmacists are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews for patients with ongoing health problems and improving patient safety, outcomes and value through a person-centred approach.

Our Clinical Pharmacists are also supported by a team of Pharmacy Technicians to help them ensure effective and efficient use of medicines for our patients by, but not limited to, improving repeat prescribing processes, promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.

Our Pharmacy Technicians work with our Clinical Pharmacists to ensure patients are managing their medication safely and support the safe, accurate and timely prescribing of medication in our Primary Care Network. They also provide technical and administrative support to the Clinical Pharmacists and Clinicians, supporting the Primary care team with audits, guidance and responding to queries around medication from patients, their carers, and other health professionals. This will include face to face and telephone support to patients, carers and primary care and community staff.
