SOCIAL PRESCRIBER

Hi, I’m Anita, I have been the Social Prescriber covering Holsworthy and Hatherleigh since July 2020. The role involves supporting adults to access community and statutory services to better meet their health and wellbeing needs. This might be helping people to access support in their own homes, helping them with housing issues or telling them about clubs and activities in their local area they might get involved with to improve their wellbeing. Through Covid, this has been challenging, with so many community and voluntary services closed. Things are opening up all the time though so now my main challenge is keeping up with what is currently available! 

I have a background in psychology, research and counselling which are all invaluable in this role. I love the challenge of finding services that meet the needs of each client or family and supporting people when they need it most. 

CAROLINE HIGGINS

HEALTH CARE ASSISTANT - VISITING TEAM

Caroline has worked as an HCA at Stratton for many years and is now part of the support being offered to people in care homes or who are medically unable to come in to be seen at a surgery.  The HCAs ensure all care home patients with a long term condition, receive their annual reviews, ensuring that each resident has a personalised care and support plan in place. The team are supported by practice GPs as required and are overseen by an Advanced Nurse Practitioner.

PARAMEDIC

Chris works across all five sites, mainly seeing people in their own homes. 

As a paramedic, Chris is able offer urgent telephone triage or care support to people unable to get into our practices, as well as helping the health management of people in care homes. He supports the work of the Home Visiting teams, and is also able to see minor ailments and injuries in the surgeries as well.

Hello! I'm Dawn Bauling working in a new role as PCN Media Officer. I have worked for the NHS since 2009, at Holsworthy Medical Centre & RCMG since 2013 in various roles and, most recently as one of Social Prescribers for the PCN. Throughout that time I have also been responsible for social media at RCMG, its monthly newsletter, website and general patient communications, so am delighted to be extending the scope of those media platforms out to all the five practices. I'm hoping to keep all our patients as informed and up-to-date as possible about local and national healthcare support and information.

Hello, my name is Helen Jackson and I am the Health and Wellbeing Coach for the Coast & Country PCN. I have been in post since December 2020 and my professional background is as an NHS Occupational Therapist. As a Health and Wellbeing Coach I work with anyone age 18 and above who has been diagnosed with a long-term health condition or is deemed at high risk of developing a long-term health condition. I offer up to six 1:1 sessions to help you work towards your own health and wellbeing goals. Each session will focus on what is most important to you! Areas to discuss could be: 

· Managing some of the challenges that a long-term health condition can present 

· Gaining further understanding about your health conditions and how to tackle them 

· Linking in with reliable sources of health information

· Setting goals around health-related behaviours such as nutrition, exercise, alcohol, smoking 

· Working through the challenges we face when setting out to achieve these goals  

I am also a qualified stop smoking advisor! 
 
I would love to hear from you if you think that health coaching could be helpful. It doesn't matter if you are unsure of what might help, this is something we can take time to explore.

CANCER CARE COORDINATOR

Hi, I’m Jennifer and I have been in post as the PCN Cancer Care Coordinator since July 2021. I help the practices in the PCN to meet their targets and improve the quality of care around cancer, helping patients diagnosed with cancer navigate their care and treatment journey. This involves providing patients with necessary non-clinical information to help with care planning, information access and emotional support services. Currently I am working in collaboration with Bude and Stratton Town Council to implement the first affordable cancer transport service for Cornwall, helping patients to get to and from hospital appointments.
My background is primarily in data and statistics, but I am extremely passionate about cancer care, having undertaken training with Macmillan to become a Community Cancer Champion alongside Age UK.

Jo works across all our sites. As a Pharmacy Technician, she complements the work of the Clinical Pharmacist, applying her pharmaceutical knowledge in tasks such as audit, discharge management, prescription issue, and keeping patients informed about medication issue and supply as needed.

LIZA CARTMELL

CLINICAL PHARMACIST

Liza works across all five sites. For more about her role press HERE.

A Clinical Pharmacist is able to clinically assess and treat patients using their expert knowledge of medicines for specific conditions. They take responsibility for patients with chronic diseases (diabetes, heart disease, COPD etc.) and undertake medication reviews to manage people with complex medication use, especially elderly patients, people in care homes and those with multiple health conditions (co-morbidities).

 

LOUISE BIRCH

SOCIAL PRESCRIBER

Louise is a social prescriber and works at Neetside and Stratton Surgeries for Coast & Country. Through discussion, social prescribing empowers people to take control of their mental and physical wellbeing, and individual circumstances, by giving time to talk and focusing on “what matters to them”.

Louise has worked as a physiotherapist, housing support worker and drug and alcohol recovery worker and this experience has enabled her to recognise a person-focused and holistic approach, encouraging connection - all of which are important in the development of social interconnections and how everyone can support each other.

Louise is a keen cold water swimmer, who enjoys nature and the pleasure it brings.

Louise works fill-time and is based at Neetside Surgery and Stratton Medical Centre.

Philippa Ogborne-Cook

MENTAL HEALTH PRACTITIONER

Philippa (or Pip) has had a long career in acute mental health nursing, working at Plymouth Hospital and latterly with the home treatment team there. She now works 4 days across the PCN covering conditions such as: anxiety, bipolar disorders, depression, personality disorder, grief reactions and trauma, amongst others. Her role provides mental health support and advice to those over 18, aiming to reduce the risk of relapse or further deterioration of existing conditions. This is not a counselling role and does not replace any work being done by the Community Mental Health Teams. 

Pip started with the PCN in 2020.

Tracey joined our PCN in April 2021, and brings with her many years’ experience in a variety of mental health settings both in the UK and Australia. Interestingly, Tracey started her NHS career in the old Holsworthy Health Centre when it was situated in the town. She started as Office Junior. She is enjoying returning to her roots and working back in the community she grew up in.

In her spare time, Tracey enjoys working out, walking in the beautiful countryside, and doing a little bit of Amateur Dramatics.

Vicky started her new role in April 2022. She started with Coast & Country in July 2021 as Vaccination Care Co-ordinator, overseeing the very successful rollout of Covid vaccinations to patients across the five practices. More from Vicky about her new role later.
